Smoking and vaping during clear aligner treatment raises practical questions about damage to the aligners, staining, and oral health. Here is what you need to know if you are a smoker or vaper considering or currently undergoing treatment.
The Short Answer
Smoking or vaping with clear aligners in is strongly discouraged. The heat, chemicals, and residue involved cause problems for both the aligners themselves and your oral health during treatment. If you do smoke or vape, doing so with aligners removed is significantly better than doing it with them in.
What Smoking Does to Clear Aligners
Cigarette smoke contains tar and nicotine, both of which stain surfaces on contact. Clear aligner plastic is porous enough to absorb these compounds, and the result is rapid, irreversible discolouration. Aligners that begin as virtually invisible can yellow noticeably within days of being exposed to smoke.
Beyond staining, the heat from a lit cigarette held close to the mouth, combined with the warm smoke passing over the aligner, can slightly warp the plastic over time. Even subtle distortion affects the fit of the tray and therefore its effectiveness at moving teeth correctly.
What About Vaping?
Vaping is generally less damaging to aligner plastic than cigarette smoke, primarily because it does not contain tar. However, many vaping liquids contain propylene glycol and vegetable glycerin, which are sticky compounds that can coat the inner surface of the aligner and create an environment for bacterial growth.
Some flavoured vaping liquids also contain compounds that can stain. Vaping with aligners in is a better outcome than smoking with aligners in, but it is still not recommended. Removing aligners before vaping is the right approach.
Impact on Oral Health During Treatment
Smoking and vaping both affect oral health in ways that compound the hygiene demands of aligner treatment. Smoking reduces saliva flow and oxygen in the gum tissue, both of which increase the risk of gum disease. Aligner treatment already requires careful hygiene, and wearing aligners for 20 to 22 hours a day while smoking increases the concentration of harmful substances against the tooth and gum surface.
Smokers undergoing orthodontic treatment also tend to experience slower tissue adaptation, which can in some cases affect how efficiently teeth respond to the forces applied by aligners.
Practical Advice for Smokers
If you smoke or vape during treatment, remove your aligners first. Rinse your mouth before reinserting them, and brush if possible. This minimises the amount of residue that gets trapped between the aligner and your teeth.
Keep your aligners clean daily using a soft brush and cool water, or a dedicated aligner cleaning solution. This helps prevent residue build-up on the tray surface.
Be aware that consistent removal of aligners to smoke can affect your daily wear time. Aligners need 20 to 22 hours of wear per day to track correctly. If frequent breaks for smoking are reducing this significantly, discuss it with the Jawology clinical team.
Frequently Asked Questions
Will smoking stain my clear aligners?
Yes. Cigarette smoke is one of the fastest ways to permanently stain clear aligner plastic. The tar and nicotine in smoke absorb into the material quickly and cannot be cleaned out. Smoking with aligners in will cause visible yellowing that makes them far less discreet.
Can I vape with clear aligners in?
It is strongly advised against. While vaping is less damaging than cigarettes in terms of tar staining, the sticky compounds in vaping liquid can coat the inside of the aligner and create hygiene issues. Remove aligners before vaping.
Does smoking affect how well clear aligners work?
Smoking affects gum tissue health and can in some cases slow the tissue adaptation that supports tooth movement. The bigger practical concern is wear time, as frequent breaks to smoke reduce the number of hours the aligner is actually doing its job each day.
What should I do after smoking if I have aligners?
Rinse your mouth thoroughly and brush if possible before reinserting your aligners. This reduces the amount of residue and bacteria that gets trapped between the tray and your teeth.
